Faculty, please report all cases of Academic Dishonesty through the form below. This is a new form and should be used solely for incidents of academic dishonesty.
Once you submit the form, the Associate Dean of Students/Director of Student Conduct will check for prior violations and contact you with the appropriate next steps. The Associate Dean/Director of Student Conduct will either forward the Adjudication Forms to you for you to adjudicate the case yourself, or request additional documentation of the violation to hear the case within the Office of Student Conduct.
