Steps for Receiving Services

I.      Fill out the Online Voluntary Declaration of Disability Form

This form is located online at https://www.sta.georgiasouthern.edu/vdd/ . Complete the form and gather the required documentation; after filling out form, the website will give you specific instructions regarding documentation that you will need to print and mail to:

Remember: You will be mailing two pieces of information. Your information form (Voluntary Declaration of Disability) and the required documentation (which may be several parts).

 

II.     Review  

          When we receive your documentation, it will be reviewed for compliance with Board of Regents requirements.

 

III.    Notification

          Following the review of your documentation, we will send a letter notifying you of the results of the review. You will receive 1 of the following letters:

          A letter stating your documentation does meet Board of Regents (BOR) requirements, and listing the accommodations you are eligible to receive,

          OR

          A letter stating your documentation does not meet Board of Regents (BOR) requirements and an explanation of what you must do to meet BOR in order to obtain accommodations.

 

IV.    Initiation of Services

          Accommodations will not be put in place until you contact the SDRC to make an appointment to arrange a meeting to initiate services.
