Steps for Receiving Disability Services at Georgia Southern University

 

Step One:    Fill out the Voluntary Declaration of Disability

This form will be in your admissions packet. Please complete it and return it to the Student Disability Resource Center (SDRC) at Georgia Southern.

Step two:    Documentation

When the SDRC receives your Voluntary Declaration of Disability, we will mail you documentation requirements. Send us documentation of your disability that meets those standards.

Step three:  Review

When we receive your documentation, it will be reviewed for compliance with Board of Regents requirements.

Step four:    Notification

Following the review of your documentation, we will send you a letter notifying you of the results of the review. You will receive one of the following letters:

A letter stating your documentation does meet Board of Regents requirements, and listing the accommodations you are eligible to receive,

or

A letter stating your documentation does not meet Board of Regents requirements and an explanation as to why it does not.

Step five:    Initiation of Services

Accommodations will not begin until you contact the SDRC and arrange an appointment to initiate services.
