Accommodation Letter

Following the completion of documentation requirements, students meet with an SDRC staff member for an intake and preparation of an Accommodation Letter. The Accommodation Letter includes:

  • Statement concerning confidentiality

  • Student's name and the last four digits of Eagle ID number

  • Date student receives Accommodation Letter

  • Semester for which accommodations are to be provided

  • List of student's academic strengths (accommodations are based on these strengths)

  • List of academic areas impacted by disability. Cognitive processing (the means by which the brain processes information) deficits determine what academics are impaired.

  • List of academic accommodations

  • Information on test proctoring
